Do I have to have a physical to play in a sport?

Physical Exam Requirements for Belding Area Schools Student Athletes

SECTION 3—Physical Examinations
SECTION 3—No student shall be eligible to represent a high school for whom there is
not on file in the offices of the superintendent or principal or athletic director of that
school, statements for the current school year certifying that (1) the student has passed a
physical examination and is physically able to compete in athletic tryouts, practices and
contests and (2) effective Aug. 1, 2008, there has been consent for disclosure to the
MHSAA of information otherwise protected by FERPA and HIPPA for the purpose of
determining eligibility for interscholastic athletics.

Interpretations—SECTION 3
16. The physical examination form must be signed by the M.D., D.O., Physician’s Assistant,
or Nurse Practitioner who administers the physical examination.

17. A school may use a locally created physical examination card and consent form or
MHSAA Form A in complying with this Section; but in all instances there must be at least
(a) a signed statement by the medical examiner (M.D., D.O., Physician’s Assistant, or
Nurse Practitioner) that the student is physically able to compete in the athletic activity
concerned and (b) effective Aug. 1, 2008, a signed statement by the parent or guardian or
18 year-old student for consent to the disclosure to the MHSAA of information otherwise
protected by FERPA and HIPPA for the purpose of determining eligibility for interscholastic
athletics.

18. Athletic equipment should not be issued and students must not be allowed to try out or
practice until an acceptable signed statement of physical examination and consent has
been provided. Consent form effective date is Aug 1, 2008.

19. A statement for the current school year is interpreted as any physical examination given
on or after April 15 of the previous school year.

20. In cases of serious injury or extended illness, students should be re-examined by a physician
(M.D. or D.O) before again being allowed to compete.

How do I enroll my child in school?

To enroll a new student, the parent/guardian needs to contact the school in which the child will attend. An enrollment packet will need to be completed for each child (available online). Parents/Guardians also need to bring proof of residency, the child’s birth certificate, and a copy of the child’s immunization record. If a family enrolls during the summer when the school buildings are closed, they should contact the Central Office at 616.794.4700.

How do I register to become a substitute teacher or paraprofessional?

All substitutes need to register through the Ionia County Intermediate School District at 616.527.4900. You may register to substitute for the entire county or specify to substitute only at Belding Area Schools.

How will I know if school is delayed or closed for the day?

The decision to cancel school rests with the Superintendent of Schools. School delays/closings will be communicated on various media sources including WZZM 13, WOOD TV8, and local radio stations. The delay/closing will also be posted on this web site.
